Intake Agreement

We require all animals to be up to date on the following vaccines / services for the health and safety of all of the animals we take care of: Rabies, Distemper, Bordetella, and a negative fecal test

Portsmouth Animal Hospital is a flea free facility. If my pet is found to have fleas at the time of arrival, I understand that a flea treatment will be provided at cost

Being away from home can be a stressful experience for some pets. I give permission for treatment and assume payment if my pet becomes ill while boarding.

Pick up times are during business hours only.

I acknowledge that should my pet become ill, a Portsmouth Animal Hospital veterinarian may provide all medical and surgical treatment deemed necessary in the doctor’s professional judgment. I acknowledge that in the event of my pet’s illness, the Portsmouth Animal Hospital staff may not be able to contact me immediately and is therefore authorized to initiate appropriate treatment until I (or the pet’s agent) can be reached. I agree to pay all related expenses associated with treatment of my pet until I am available to discuss further care and related fees with the attending veterinarian.

All reasonable precautions will be used to prevent injury and escape of the pet. Portsmouth Animal Hospital is not responsible for the actions of the pet that may cause injury and escape.

I will notify the hospital of any change of plans that prevent me from picking up my pet at the scheduled reservation pick-up date. If I do not pick up the animal within ten (10) days of the scheduled reservation pick-up date, Portsmouth Animal Hospital will assume the animal is abandoned. If the animal is abandoned, Portsmouth Animal Hospital is authorized to remedy the abandonment as prescribed by law. I further understand that abandonment DOES NOT release me of my financial obligation for services rendered, fees associated with abandonment, collection action, and/or legal services.

Excellent establishment! Not only are the receptionists all so friendly and accommodating, but they got my poor old boy in almost immediately because he was in pain. Also, Dr. Smith and Leah were absolutely amazing! They are friendly and compassionate and obviously love and are blessed at what they do...thank you all so much!
